Mosaic in Axtell is an Intermediate Care Facility for the Mentally Retarded (ICF-MR) that offers residential and day services in a 24-hour assistance setting. We provide services that respect the human dignity and rights of all and are tailored to each person's needs, wants and interests.
An interdisciplinary team of family, staff members and professional consultants support individuals served in developing personal goals and programs. Mosaic's services help people participate fully in Axtell's community life.
