If you are not currently receiving any services, contact your local MRA (Mental Retardation Authority) and ask for Intake. You can find your local MRA through the Texas Department of Aging and Disability Services. Mosaic in Dallas works with Dallas MetroCare Services, Denton County MHMR and Lakes Regional MHMR MRAs. Make sure your MRA assists you to be placed on all waiting lists (for example, HCS, CLASS, MDCP, TxHmLv Waiver, etc.).If you have gone through Intake
ICF - MR Program
If you have already gone through Intake at your local MRA and are interested in ICF - MR Program, please use our Information Request Form and designate ICF - MR, or call Jane Shedd at 972-866-9989 x319.
To be placed on our waiting list, please mail a letter stating your interest along with a packet of personal information to:

Please make sure and include such items as a psychological evaluation, medical history, social history, etc.
HCS Program
If you have already gone through Intake at your local MRA and are interested in the HCS Program, please use our Information Request Form and designate HCS Program, or call Tammy Johnston at 972-866-9989 x315.
