Mosaic in Dallas provides the following supports:

- Texas Home Living Program (TxHmL)
Our HCS (Home and Community-Based Services) program has one residence for three people and provides 24-hour supports. Additionally within the HCS program, we provide Supported Home Living supports and Foster Care supports.
Our Vocational Center is located in Carrollton and currently serves more than 55 people. The Vocational Center provides training to teach people vocational skills and an opportunity to earn an income through completion of contract work as well as the Document Destruction business.
For people either not eligible or wanting HCS and/or ICF-MR services, we will provide Case Management, Vocational and other services for people with disabilities as desired by private pay arrangements.

The TxHmL Program provides services and supports to Texans with intellectual disabilities or a related condition so that they can continue to live with their families or in their own homes. There is no age limit for this program.
A person is eligible for the TxHmL Program if he or she:
- chooses to participate in the TxHmL Program instead of the ICF (Intermediate Care Facility) Program
- lives with their family or in their own home
- is diagnosed with an intellectual disability or related condition and meets the criteria for a Level of Care 1 in an ICF
- is a Medicaid recipient
- is not assigned a pervasive plus level of need (LON 9)
- is not enrolled in another Medicaid Waiver Program, such as HCS Program, CLASS Program or Medically Dependent Children Program
TxHmL Program services are intended to supplement, rather than replace the services and supports a person may receive from other programs. TxHmL Program services are limited to a yearly cost of $17,000 per participant. TxHmL Program does not offer residential services.
If you are on the HCS Program interest list (waiting list), you may still receive TxHmL Program services and your name will stay on the HCS Program interest list with no change to the date it was put on the list.
There are two service categories with a combined yearly cost of not exceeding $17,000.
TxHmL Program service categories and service components include:
Community Living Supports
annual service category limit is $13,600
- Community support
- Day habilitation
- Employment assistance
- Supported employment
Professions and Technical Supports
annual service category limit is $3,400
- Skilled nursing
- Behavioral support
- Physical and occupational therapy
- Speech and language pathology
Minor home modifications (These are limited to a lifetime maximum of $7,500. Once lifetime maximum is reached, the Department of Aging and Disability Services (DADS) allows $300 per year to be used for additional modifications or repairs of modifications.)
Adaptive aids (limited to 6,000 per year and can not be increased more than that)
Dental treatment (limited to $1,000 per year and can not be increased more than that)
When your need for a service or a combination of services in a service category is greater than the annual service category limits, DADS may approve a request to increase the service category limit as long as the total annual cost of your plan of care does not exceed $17,000. You, your representative, or your TxHmL Program provider can let your service coordinator know that you need to increase a service category limit.
