Mosaic in Central Iowa partners with people with intellectual disabilities to create a meaningful life in a caring community, giving a voice to their needs.

Mosaic in Central Iowa provides the following supports:

  • ICF/ID - Intermediate Care Facility for persons with intellectual disabilities. This service level provides 24-hour residential supports and supervision. Meals, transportation, medical service coordination are included in this service.
  • Home and Community Based Services (HCBS) - We provide 24-hour daily residential supports and supervision in homes, apartments and host home settings.

  • 24-Hour Supported Community Living (SCL) in Group Homes & Apartments - Persons served can choose an apartment or group home where staff provide supports that foster more independence in home management, financial skills, recreation and leisure activities.

  • 24-Hour Supported Community Living (SCL) in Host Homes - Host homes match a person receiving services with someone who provides the assistance needed for daily living in a family-like or roommate-like setting. The program is designed to allow an individual to enjoy the benefits of family living while continuing habilitation and personal growth.

All services are available to adults (age 18 and older) with intellectual disabilities. Funding for services is based upon eligibility for Medicaid or can be privately paid.

Admissions coordination is handled by Mosaic's Admissions Director. For more information or to begin the admissions process, contact us and someone will be in contact with you to provide you with an application. Once an application is complete, a meeting will take place to determine level of services needed and the ability of Mosaic to meet those needs.
