Services

Mosaic in Des Moines provides services to match the individual needs of each person we support. Our services include:

ICF/MR-Intermediate Care Facility for persons with mental retardation. This service level provides 24-hour residential supports and supervision. Meals, transportation, medical service coordination are included in this service.

Home and Community Based Services (HCBS) and Supported Community Living (SCL) services can provide hourly as well as daily residential supports and supervision in home, apartment or family home settings.

Pre-Vocational services provide supports to help someone gain skills for employment.

Supported Employment services will assist someone at an actual job site.

All services are available to adults (age 18 and older) with a primary diagnosis of mental retardation. Funding for services is based upon eligibility for Medicaid or can be privately paid.

Admissions coordination is handled by Mosaic's Admissions Director. Use our Information Request Form and check admissions to receive an application for services. Once an application is complete, a meeting will take place to determine level of services needed and the ability of Mosaic to meet those needs.

We invite prospective clients, families and guardians to visit Mosaic's residential and employment sites prior to making a decision to receive services from Mosaic.
