Services
Mosaic in Ellsworth provides the following supports:
- Residential Services - Mosaic offers a wide array of residential settings in the area to accommodate the needs of each person best.
- Group and Supported Living Services - Residences in which 19 individuals live in homes owned by Mosaic. These settings encourage independence and community integration while providing professional and caring services by Mosaic staff around the clock.
- Independent and Semi-Independent Living Services - Delivered in settings ranging from a shared home or apartment to the person's own home. Services are developed based upon the individual needs of the person or people living in that home.
- Host Family Homes - Settings where people live with families. This program allows a person the support of a family and the support of Mosaic while continuing in personal growth.
- Case Management - Provides information about services and supports, then assists in accessing what is needed through the development of a person- centered support plan. Targeted Case Management is available to qualifying adults and children in Ellsworth and surround counties.
- Day Programs - Mosaic focuses on a meaningful day experience by offering programs specific to the needs of each person.
- Adult Life Programs - Provides training to enhance everyday living skills, accessing the community, pre-vocational training, and volunteer opportunities. Our program encourages progress towards individualized outcomes and goals. A Computer Lab and Sensory Therapy are available.
- Center Based Employment - Allows people to work on various jobs at the main site including manufacturing buttons, door mats, fire starters, and preparing items for Mosaic's own thrift store, Hidden Treasures.
- Enclaves - Allows people to gain solid and realistic employment experience in area businesses. Individuals work for companies with the help of Mosaic staff.
- Supported Employment - Helps people take the final step toward vocational independence. Job coaches work with individuals and businesses to locate and secure employment. Job coaches then continue to provide the needed communication or support for people working in the community.
