Services
Mosaic in Garden City provides the following supports:
- Case Management - Coordinates the supports for clients who receive supports from Mosaic and other service providers.
- Child Services - Services provided to people under the age of 21. Supports may include residential services, supported/intermittent, host home, day services, special education or early intervention.
- Day Services - Services comprise any vocational services, supported employment, community participation or habilitation training.
- Host Homes / Foster Care - Services provided by an independent contractor.
- Residential Services - Services provided 24 hours a day, seven days a week. Both Intermediate Care Facilities/Mental Retardation (ICF/MR) and waiver residential settings are included. Generally these services are provided in an agency-leased or agency-owned facility.
- Supported / Intermittent - Services provided few than 24 hours per day and can be provided in a home owned or leased by the client or the agency. Supports can vary from a few hours a week to several hours a day. These services are provided through a waiver facility and usually are based upon the needs of each person.
- Affordable & Accessible Housing - Providing people with and developmental disabilities with quality, affordable and accessible housing is a priority for Mosaic. By partnering with federal, state and local funding sources, Mosaic is able to provide quality homes at a reduced cost to the organization and residents. Mosaic is then able to direct more resources to other necessary services for people with and developmental disabilities.
Mosaic builds low income units in residential neighborhoods. We have built group homes, duplexes and multiplex buildings that are close to grocery stores, bus stops and other community services.
People who apply to live in Mosaic housing do not have to receive other services from Mosaic and may receive services from other providers. All residents must qualify according to established income guidelines. Income must be below the low or very low-income guidelines for the county of residence.
