Intermittent Services:

Life Assistance Solutions Private Pay Option
Mosaic's trained, caring staff can fill in when family members or friends aren't able or available to help. This is not assisted living or medical care, but instead provides help with household tasks, or maybe just companionship or an escort for shopping and errands.

Supported Living Services

To link services and supports with existing natural supports and generic community services so that medicaid eligible adults, who are responsible for their own living arrangements, have the necessary supports to be included in typical Colorado community life. Supports may includes items such as grocery shopping, assistance with medications, medical care coordination, and group activities. 

Children (CES)
To provide Medicaid benefits and additional targeted services and supports to children with developmental disabilities or delays who live with their families and who are most in need because of the severity of the disability. These include behavioral supports, community connector, and respite.

Residential Services:


Host Homes

Host homes match a person receiving services with someone who provides the assistance needed for daily living in a family-like or roommate-like setting. The program is designed to allow an individual to enjoy the benefits of family living while continuing habilitation and personal growth.

Semi-independent Services

We assist individuals living in their own residences with daily living activities (i.e.. money management, cooking, shopping) or socialization activities. This is accomplished by identifying the frequency of contact that is most appropriate for the person and their needs.

Meaningful Day Services:

Specialized Habilitaiton
Generally services are provided in a training center/class room with others that have intellectual disabilities. Activities include games, crafts, classes, exercise, and engaging events that are similar to a retirement program or class room.

Community Connections
Services are provided in typical community-based settings enjoyed by all citizens. We partner with other community non-profits needing volunteers and provide a small group of individuals out to give back to their community. Some of our partners are Salvation Army, Goodwill, and local animal shelters.

Supported Employment
To provide vocational services which promote and support inclusion within the community. We encourage and assist people in obtaining jobs in the community or running their own small business. Our focus is on assessing interest, job placement, and job coaching.
