Mosaic in Northern Indiana provides the following supports:

• Group Homes - People Mosaic serves reside in five residences in Elkhart and South Bend. Supports are geared to assist people in becoming more independent in home management, vocational skills, accessing community resources, transportation, and recreations and leisure activities. Supports are individualized to assure each person is capable of growing and learning as they are exposed to new opportunities.
• Supported Living - These services are provided through various Home and Community Based Waivers. Services include but are not limited to residential habilitation and support, respite, and transportation. Some people receive full time supports and others receive intermittent services. Some are supported in their own apartment while others continue to live with family and friends.
