Host Home Provider
This position affords the opportunity for members of the community to earn an income by opening up their home to an adult with a developmental disability. Providers maintain an independent contractual relationship with Mosaic. The people served in host homes may have a range of care needs, varying from 24/7 care to being independently employed in the community.
Providers can expect daily paperwork, regulations, support, monthly monitoring and attendance at professional meetings. Specialized training will be provided and a Mosaic manager would be assigned to the provider.
This individual must be able to provide clean, safe and suitable home for the person and assure availability of safe and reliable transportation. Must be able to obtain required training in regard to the individual's needs in accordance with state rules, regulations and Mosaic policies and procedures. Must have a clear criminal background and acceptable driving record.

direct support associate
Direct Support Associates are responsible for training and assisting individuals served in various aspects of activities of daily living while ensuring compliance with regulatory requirements and company policies and procedures.
This position is also responsible for the health, safety and emotional support of individuals being served.
High School diploma or equivalent required. Minimum of three months of experience in a related field required. Must be at least 18 years of age or older.

direct support manager
The Direct Support Manager coordinates and manages program operations while ensuring compliance with regulatory requirements. Responsibilities include hiring, training, and scheduling staff, coordinating activities and appointments of individuals served, and maintaining records.
This individual must possess a Bachelor's Degree in a related field and one year of experience. Position may require CMA permit, CPR and first aid certification.

habilitative specialist/case manager
The Habilitative Specialist/Case Manager directs and coordinates the delivery of habilitative services in compliance with regulatory requirements. Responsibilities include training staff in behavior modification techniques, developing new programs and maintaining records.
Bachelor's Degree in Psychology, Special Education or related field required. Minimum of one year of experience working with individuals with developmental disabilities required. May require QMRP certification and experience in an ICF/MR.
