home and community based services
Group Homes - Mosaic operates two ICF/MR group homes in Osceola. Each home provides private bedrooms for the seven individuals who live there. Staff members are present around the clock and provide support in home management, vocational training, medical needs, social skills and community inclusion.Supported Community Living - These services vary from hourly to daily supports based upon individual need. Many individuals supported live in their own apartments while others may live with their family.
Respite - Respite services are designed to give an individual's primary caregivers a break while ensuring their loved one is well cared for. Respite services are generally provided in the caregiver's home.
vocational/day services
Meaningful Days - This service provides a wide variety of daily activities for individuals who do not work in the community. Activities can be provided on-site or within the community and may include volunteer opportunities, horseback riding, social time, cooking classes, crafts and games. Staff are available to help each individual with his/her medical and personal care needs.Vocational Training - Mosaic realizes that people with disabilities can be productive within the workforce. Staff encourage individuals to learn and improve their work skills and work habits at our on-site vocational training centers. Each individual earns money based upon their personal work level and productivity.
Supported Employment - Mosaic assists individuals to acquire jobs of their choosing within the community. Support is also provided to help the individual learn and maintain the job.

That's why our host homes reflect a great variety of personal choices and lifestyles in communities throughout South Central Iowa.Through our Mosaic Host Home Program, individuals with disabilities are carefully matched with a host home family. In the nurturing environment of a family home, people with disabilities receive assistance with activities like daily living, social skills or personal care needs. While in the home, a continual learning process takes place, not only for the individual, but for the family as well.
