Available Services

Mosaic in Phoenix helps people with development disabilities through four distinct programs:

HOST HOME SERVICES: Offered in a personal, 24-hour home setting. Host Home services are designed to match children, teens, and adults with caring individuals and families. Available Valley-wide.

EARLY INTERVENTION PROGRAM:Working closely with parents, our team of Early Interventionists provide in-home therapeutic activities to promote healthy development for young children birth to three years old. Available Valley-wide.

RESIDENTIAL PROGRAM: 24-hour support is provided in six attractive adult residential homes. Through guided assistance, residents learn practical life skills and receive the personal care needed for optimum quality of life. Located in Central/Northwest Phoenix.

HOME & COMMUNITY BASED SERVICES: Customized to meet the needs for people of all ages, this service brings a compassionate worker to assist in the privacy of your home. Respite, habilitation, and attendant care are available Valley-wide.
