Searching for IDD services? Ask these 6 Questions to Find the Right Provider

Choosing a provider for intellectual and developmental disability (IDD) services is an important decision. It can also be overwhelming.

Whether you’re a family member, caregiver or self-advocate, asking the right questions is key to finding a provider that aligns with your specific needs, goals and dreams.

Here are six questions to ask when searching for IDD services:

1. How long have you been in business?

Longevity can be a key differentiator when it comes to choosing a provider. A long-standing provider brings refined expertise, institutional knowledge, which helps ensure operational stability, reducing the risk of service disruptions or closures. Mosaic has been serving the IDD community for more than a century. In that time, we’ve evolved to meet people’s needs and expectations, but our purpose has stayed the same: We are called to love and serve.

2. What services do you provide?

The right provider is one who fits your needs, so make sure they offer the services that you are looking for. If your loved one has a complex medical need, ask the provider if they have experience supporting someone with that specific need (e.g., G-tubes, tracheostomies, mobility challenges, etc.).

Mosaic offers a wide range of services—24/7 supported living, hourly supported living, day services, employment skills and mental and behavioral health and autism services—that enable people with varying needs to thrive in their communities and accomplish their dreams and goals. Our largest service line is Mosaic at Home. Through Mosaic at Home, we match people with shared living providers who are a great fit for their needs, interests and lifestyle.

3. Is your organization person-centered?

Person-centered care places the individual, their values and preferences at the heart of care decisions, moving away from a “one-size-fits all” approach. People want a provider who is a partner in their care. If the provider answers “yes” to this question, ask them to provide a specific example of a goal they helped a person they support achieve.

Mosaic’s personalized services empower people with intellectual and developmental disabilities and other diverse needs to live their best lives. When a person begins services with Mosaic, we spend time understanding what a good life looks like to them and find ways to make it possible. We use  Personal Outcome Measures as a framework to understand what’s important to the person.

4. How will you make sure my loved one has a consistent, familiar face providing their care?

For many families, consistent care is important. While the industry faces high turnover, Mosaic is moving in the opposite direction.

Recent data showed a 17% reduction in Direct Support Professional turnover between 2021 and 2024. Also in 2024, our employee Net Promoter Score, a metric used to measure customer loyalty and experience, rose by 12 points, proving that our staff is as committed to Mosaic as they are to the people we support.

In our Mosaic at Home service model, your loved one lives with a dedicated provider who becomes a consistent, familiar presence in their daily life. Instead of rotating staff, they build a stable, trusting relationship with someone who truly knows them.

5. Are you part of a larger network of care?

Being part of a larger network has some major advantages. At Mosaic, our National Supports Team takes care of back-end support, such as finance, compliance, human resources, fundraising and more. This means our local staff can focus entirely on supporting the individual.

Being part of a large network also means we can share great ideas with and learn from colleagues across the country so we can give people the best care possible.

6. How will you ensure my loved one receives quality health care and support?

You want the best for your loved one, and you deserve a provider who feels the same way. Asking about the provider’s commitment to quality will give you valuable insight for your decision making. At Mosaic, we have a Health Services team that keeps a close eye on health trends and ways to improve your loved one’s care, when they need it.
