Mosaic has offered Host Homes for more than 40 years. The service matches a person who needs support with a Home Provider. The Home Provider not only offers a place to live but also gives 24/7 support tailored to individual values.

We call our Host Homes service Mosaic at Home®, but it is so much more than a home.

More Than a Home

Host Homes provide the right combination of belonging, independence, and the opportunity to make connections. It starts with an extensive matching process in which people choose where and with whom they live. Both the person supported and the home provider choose one another, and matching suggestions are based on interests, values, lifestyle and personality.

For those who value quiet comfort and routine, the right match ensures people feel not only welcome, but truly cared for. More than a home, it is a place to belong.

People served through Mosaic at Home receive support for their personal healthcare needs related to chronic illness and/or disabilities.

Mental and Behavioral Health and Autism Services

People receive compassionate, evidence-based applied behavioral services based on their unique needs. Services foster whole-person growth toward individual success and improved quality of life.

Group Setting

Three or more people live in a shared setting where support staff are present 24/7 to help people acquire, retain or improve skills related to the activities of daily living.

Supported Living (24/7)

People living independently or in a family setting have in-person or virtual access as needed to Mosaic support staff 24/7 to assist with personalized, individual needs.
